Submit the UCC-11 Information Request Form along with:

• $5 per Debtor/Taxpayer name (plain certification)
• $5 + $10 per Debtor/Taxpayer name (gold seal certification)

After search results are returned, then request:

• Copies - $1 per page (or print copies for free online)
• Certified copies - $10 + $1 per FTL page (gold seal certification)
• Filing fees are non-refundable
